As a Care Coordination Associate, your primary responsibilities will include:

**Your Key Duties:**

As a Care Coordination Associate, you'll be entrusted with pivotal duties that directly contribute to our clients' well-being and satisfaction. Your key duties will encompass:

Initiating new claims and securing necessary medical requirements.

Managing the recertification of permanent, non-recoverable claimants.

Updating and optimizing clients' Plans of Care.

Coordinating with hospice providers to maintain eligibility.

Providing ongoing support to clients through calls, inquiries, and claim invoices.

**Qualifications:**

- High school diploma or GED required.

- 2+ years of prior long-term care or insurance support experience preferred.

- Proven customer service or customer relations skills.

- In-depth knowledge of long-term care insurance.

- Proficiency in Microsoft applications and strong computer literacy

- Strong organizational and decision-making abilities.

- Exceptional written and verbal communication skills.

- Capability to foster strong working relationships and excel in a team environment.

- Completion of America's Health Insurance Plans (AHIP) Long-Term Care courses within 3 years of joining.

- Must be within driving distance to Portsmouth, NH as this is a Hybrid role requiring 2 in-office days (Mon/Tues or Thursdays preferred)
